to complete the flight that day? If so, identify
that as a hazard.
If we tried to remove all threats
from life, we couldn't go anywhere
or do anything, and even then,
there would be a risk. So, we aren't
trying to eliminate all risks.
After the hazards for each flight are identified,
they need to be assessed for their level of risk.
We know that the complete elimination of risk
is impossible. There is risk in everything we
do from the moment we step out of bed in the
morning. If we tried to remove all threats from
life, we couldn't go anywhere or do anything,
and even then, there would be a risk. So, we
aren't trying to eliminate all risks. What we
need to do is assess the threat level that each
of the hazards presents, the probability that an
event will occur, and the severity of injury or
damage expected. Then we need to find ways to
bring risk to an acceptable level.
Let's assume you've identified a hazard as a line
of severe thunderstorms between yourself and
your destination. If you were to penetrate that
line, what would the severity of such a situation
be? The risk would be an aircraft accident due
to loss of control or flight into terrain, either
of which could be catastrophic. So, would you
willingly penetrate a severe thunderstorm that
day? Deciding to fly into a severe thunderstorm
would put you solidly into the high-risk category.
The FAA created a risk assessment table
(see sidebar) for pilots to ascertain a trip's
combined risk and mitigation level. If a hazard
is assessed as a high (red) or serious (yellow)
risk, it implies the flight is a no-go, and you
must mitigate the risk before departure or,
if in flight, divert or take other appropriate
action. For medium (green), the flight can
depart or continue, but the risk should be
reduced whenever possible. For low (white),
the risks can usually be mitigated by following
checklists and standard operating procedures.
There will always be more than one hazard
present, and hopefully, they will each be
identified, assessed, and mitigated before
every flight. Each category of PAVE will likely
identify hazards. Maybe last night you didn't
get a good night's rest. Poor sleep quality would
be a hazard that could lead to fatigue-related
errors and could be categorized as critical in
