be a high (red) risk. Perhaps you determined
that the aircraft can't carry full fuel for weight
and balance reasons and assessed the risk of
fuel exhaustion as a remote likelihood with
catastrophic severity as a serious (yellow) risk.
As much as we think we can do
this identification, assessment, and
mitigation in our heads, it is much
more helpful to list the risks before
each flight to better visualize the
cumulative effects of the hazards.
For Risk Mitigation, any risks that fall into the
high or serious categories need to be addressed
before flight, and action should be taken to
reduce the risk to an acceptable level. Following
the example above, if you decide to wait until
the line of thunderstorms passes, this action
would minimize the risk in multiple areas. The
likelihood of penetrating a thunderstorm will be
reduced if you wait until it passes. If you delay
departure, you can sneak in a nap, decreasing
the probability of fatigue, and without having
to circumnavigate storms, the risk of fuel
exhaustion is less likely.
As much as we think we can do this
identification, assessment, and mitigation in
our heads, it is much more helpful to list the
risks before each flight to better visualize the
cumulative effects of the hazards. Putting them
on paper gives us a fuller perspective of the
risk picture. To accomplish this in a clear and
objective way, several companies have developed
Flight Risk Analysis Tools (FRATs), including
the FAA FRAT. Some FRATs are built directly
into flight planning software and account for
weather forecasts and preset data about the pilot
and aircraft. Some are spreadsheets that lead
a pilot to evaluate each category of the hazard
before flight.
