Follow Your Instincts
When I was in private practice, there were several instances when I decided to follow
my instincts rather than pursue what would normally be the accepted treatment or diagnostic
course for a patient. We all heard in medical school, " When you hear hoofbeats,
look for horses-not zebras, " but there are times when your instincts may direct you to
chase a zebra.
I can think of one instance when a healthy middle-aged marathon runner came in
with calf pain. He had seen some other physicians and was diagnosed with Achilles tendinitis,
but it had not responded to traditional treatment. He was in great physical condition,
and his medical history and medications were unremarkable. His history resembled
intermittent claudication, and his pedal pulses were very slightly diminished on the affected
side. Normally, getting non-invasive vascular studies in a patient of this age and in
this physical condition would not make sense, but my gut was telling me something was
going on. I explained everything to the patient and referred him for non-invasive vascular
studies. It turned out he did have a blockage that was easily resolved with an interventional
surgical approach. The patient was grateful, but more importantly, following my
instincts was the right thing to do.
I think this approach applies to all aspect of our lives, not just to the treatment of
patients. There are times when for some reason, our instincts make us feel uncomfortable
with the decision that seems like the expected course. In my life, I have learned
that usually my instincts are correct and following them helps me avoid a bad decision. I
have found more times than not, my gut guides me in the right direction.
All decisions should involve a thoughtful process and consideration of the pros and
cons, but don't ignore the intangibles.
