This seminar describes the important and unique aspects of oral
care and management in patients born with cleft deformities. The
crucial role that pediatric and/or family dentists play is an integral
part of a cleft team. With the popularity of pre-surgical infant orthopedics such as Nasoalveoar Molding, their role can start as early
as prenatal counseling when parents are informed of their choices.
This seminar describes the first stages of anticipatory guidance,
creating the foundation of oral prevention and establishing the
“cleft dental home” for patients born with cleft deformity. It also
addresses the skeletal, dental, hard and soft tissue consequences
seen in the pediatric population as a result of their cleft deformity
and surgical repairs.

The management of patients with cleft lip and cleft palate requires
extended orthodontic treatment and an interdisciplinary approach
to care. This approach results in providing patients with optimal
esthetics, function and stability. Orthodontic or orthopedic management in primary, mixed and permanent dentition and following
the completion of facial growth, are discussed. The focus of this
presentation is on current concepts in the interdisciplinary approach to treatment planning and sequencing during each phase
of orthodontic treatment.

Clinical experience with Nasoalveolar Molding (NAM) and presurgical columella elongation in patients who have unilateral and
bilateral clefts of the lip alveolus and palate (UCLP and BCLP) are
reviewed. NAM addresses the deformity of nasal cartilages and
deficiency of columella tissue in UCLP and BCLP. This technique utilizes wire and acrylic nasal stents, attached to the vestibular shield
of an oral molding plate to mold the nasal alar cartilages into normal form and position during the neonatal period. The objective of
presurgical NAM is to reduce severity of the nasolabial and alveolar
deformity, enhancing conditions for a successful surgical repair.

From childhood and often infancy, the role of the prosthodontist
must be incorporated into the overall treatment goals outlined by
the cleft palate team in order to optimize care long after the treatment provided by the surgeon, orthodontist and pediatric dentist
have finished. This presentation outlines the role of the prosthodontist on the cleft palate team and reviews the advantages and
disadvantages of the various treatment modalities available in restoring the dentition of the patient with a cleft in providing the “finishing touches.” These treatments often serve to truly “complete”
the care provided over the years by the entire cleft team.
