Throughout FY15-16, HQP continued to
support community health centers and other
safety-net partners with its broad portfolio
of quality programs and services that rely
on real-time data analytics, research
and education.
INCREASED REVENUE FROM GRANTS AND CONTRACTS, EXCEEDING BUDGET BY 10%
HCP and HQP were awarded nearly $6.2 million in funding in FY15-16.
Some of these awards represent multi-year funding. Funding highlights over the past fiscal
year that have strengthen HCP members' ability to provide and enhance clinical care include:
Behavioral and Physical Health Integration: received funding for this initiative from the County
of San Diego Health and Human Services Agency Behavioral Health Services Administration and
Blue Shield of California Foundation.
Breast Health Diagnostics: with funding from Komen San Diego and Mammograms in Action /
Barbells for Boobs provided 619 services to health center patients.
Diabetes and Cardiovascular Health Services: with funding from Kaiser Permanente continued
the infrastructure to support the spread of the ALL HEART project among safety net providers
in Southern California.
Navigator Program: received a grant from Covered California for health coverage enrollment for
the period of August 2015-June 2016.
Oral Health Initiative: funded by First 5 San Diego to provide comprehensive oral health services to
pregnant women and children ages 0-5.
Pediatric Care Access: HQP, with funding from the Centers for Medicare and Medicaid Services
(through the Connecting Kids to Coverage Initiative) will work with six-member health centers to enroll
6,400 children in Medi-Cal over a two-year period, focusing on those who are newly eligible, as well as
older youth and teens.
