CLINICAL CARE COORDINATION &
QUALITY IMPROVEMENT
GOAL: Develop a member training calendar
and learning academy for FY17-18, with
cross-functional tracks.

For timely credentialing MedPOINT Management
assumed responsibility for IHP credentialing. They
also conducted patient satisfaction surveys for
member patients and completed an access audit.

In 2017 IHP began health center specific
trainings on the Medicare Annual Comprehensive
Exam (ACE). IHP continues to increase training
efforts on behalf of our members as they target
specific HEDIS metrics.

IHP staff continued to attend and actively participate 
in health center quality meetings on a regular basis. 
A goal of IHP was to optimize IT systems to reduce
costs, increase efficiency and improve the health
center and patient experience. As a result the IHP
Analytics and Quality Improvement Committee was
formed.  A committee partnership with Premier, Inc.
began in May to complete assessment interviews with
IHP staff/members and stakeholders.  The committee
reviewed the Premier Population Health Information
Technology (PHIT) report with members in June
and the IHP Board of Managers moved forward with
evaluating new technology solutions to advance the
CIN.  

IHP facilitated training by Molina Healthcare on
best practices for hypertension, as we continue to
increase our training efforts on behalf of our
members. 
GOAL: Begin operationalizing IHP, including the
major functional areas of credentialing, patient
satisfaction surveys and IT strategy.
During FY 16-17, IHP worked toward its 2020
goal to produce a market-leading Clinically
Integrated Network that would allow for member
agencies to achieve the Triple Aim. The strategies
included standardization through aggressive
quality metrics and adoption of shared clinical
protocols to improve quality. In March 2017, we
finalized Year Two Clinical Metrics.  

Finally, as a result of our dedication to the Triple Aim
we were pleased to receive CY2016 HEDIS
results, which showed marked improvement in all
metrics and exceeded past years.
