Many Americans forego
primary care because of how
inconvenient it is, thanks to
long waits, frequent cancellations, and time away from
work. Additionally, individuals
living in economically stressed
urban areas might be more
than 5 miles away from the
nearest clinic or health center,
resulting in higher rates of
preventable disease.
T H E CO N C E P T
Fare2Care ideates " care cars "
that can attach to public trains
in any city across the 8,000
miles of metro rails in the U.S.
Patients would visit the cars
to access primary care during their regular commutes, a
solution to provide healthcare
where people are. The cars are
integrated with technology to
streamline the experience and
offer comprehensive telehealth
services.

THE D ETAILS
Using San Francisco's BART
system as an example, about
600 primary care modules could
be accommodated on its fleet,
resulting in 12,000 appointments
available each day in addition to
the ability to conduct mass health
screenings, vaccinations, etc. The
Fare2Care system could even
detect elevated temperatures of
patients inside a transit station

and send alerts to seek care. In
fact, it's that type of technology integration that fuels the
solution, including an app that
patients would use to book
appointments, look up data on
previous visits, etc. Technology
in the cars themselves provides three layers of security,
while solutions like UVC light
and HEPA filtration as well as
automated cleaning systems
address infection control.

individuals by allowing medical teams to passively observe

data. G.I.S.M.O. is also able to aid

physiological and psychological
needs and stressors that present
during a typical appointment.

THE DETA I LS
Composed of thousands of
microscopic pins coded to read
and adapt to a variety of patients,
G.I.S.M.O. is responsive to
patient needs and able to transform into an infinite number of
configurations. For example, the
pins might morph into handrails
in a corridor at the exact height
a patient requires or become
seating that's adjustable and
ergonomic for that individual.
The pins can also emit sound and
light and expand and contract
while collecting and storing data.
For example, an interactive gaming experience might engage and
distract a patient while also being
used to collect information on
audio and visual capabilities, informing a patient profile and any
assistive devices needed. Inside
the exam room, two-way observation space allows physicians
to view patients as they react and
respond to the room. The data
collected through the system can
also be stored and used to inform
future patients and their diagnosis and treatment.

many causes, while some are
unknown. If not diagnosed
early and treated, these
can lead to developmental
delays. However, it can be
challenging to recognize
the subtleties of patient
processing issues in a traditional exam setting.
