good medical documentation: accurate, factual,
timely, and organized.14
CONCLUSION
Although it is difficult to measure the amount
of errors caused by healthcare workers, when a
worker has 50 patients in their charge of whom
they must take their VS, and these are handwritten
in a logbook and then captured manually in
the system, it is clear that he is exposed to many
errors along the way.
