Chapter 4-12 Passenger Plates Revocation Status If a Lottery plate is in a Revoked status (REVO) due to insurance or other reason(s), the consumer has one year from the date of revocation to clear any obligations or the Lottery plate will be cancelled and be eligible for the following year’s Lottery. Lost Lottery Plate Process The consumer must provide the following, to an authorized RMV personnel, when ordering a replacement plate(s): • Universal Registration Form • $10 Lost Plate fee ($20 fee if ordering two plates) The RMV clerk will access the RI screen to verify the customer information. Then the RH screen to inquire on the issue date of the Lottery plate. • If the Lottery plate being requested was issued less than 14 days prior to the lost plate application, the clerk will contact the Special Plates Department to receive approval to process the lost Lottery plate transaction. • If the Lottery plate was issued more than 14 days prior to the request, the clerk can process the transaction without contacting the Special Plate Department. 9/08 4:45http://www.massagent.com/rpmlinks/universalapplication.pdf
