Chapter 6-14 Fees Massachusetts ID Massachusetts ID/Liquor ID • There is a $25 fee for a Massachusetts ID. • There is a $25 replacement fee for a lost or damaged Massachusetts ID Fees Massachusetts Liquor ID • There is a $25 fee for a Massachusetts Liquor ID. • If you lose your Liquor ID, you must go to any RMV branch office and sign a form indicating it has been lost. The fee to obtain a duplicate Liquor ID is $25. Expiration: Massachusetts ID and Liquor ID expire every five (5) years. Here is a form for the Mass. ID and Liquor ID. 4/09 Documentation Requirements Massachusetts ID You must have a valid social security number or an acceptable denial letter with an acceptable visa code. If you are under 18 years of age You must present the following: • 1 document proving date of birth • Parental consent on the Class D, M, or D/M License and ID Card application. • A Social Security Number (SSN) that the RMV can verify with the U.S. Social Security Administration (SSA) as having been issued to you or an acceptable Denial Notice from the SSA.
