Chapter 6-15 Gender Designation on Driver’s License or ID Gender Designation on a Drivers License or ID Card The Registry of Motor Vehicles has revised its policy for applicants requesting to change their gender designation on a license or ID card. This policy applies to: • Applicants requesting a change of the gender designation on their current Massachusetts license or ID card; • Applicants converting a license from another jurisdiction and changing the gender designation for the new license; and • Applicants for a new license or ID card whose identity documents indicate a gender other than the gender designation requested on the new license or ID card. An applicant who wishes to change his/her gender designation must: 1. Submit an original, completed Massachusetts Gender Designation Change form signed by both the applicant and a medical or social service provider; 2. Submit a completed and signed License and ID Update application; 3. Surrender the existing license or ID card that is to be amended; and 4. Pay the applicable fees for the new or amended license or ID card. A photo-image and signature will be required for the new or amended license or ID card. These applicants do not need to provide proof of a court-approved name change, sex reassignment surgery or an amended birth certificate. 01/09 6:68http://www.massagent.com/rpmlinks/genderform.pdf http://www.mass.gov/rmv/forms/21042.pdf
