If you present a Denial Notice, you must also present proof of acceptable visa status, an I-94 (Record of Arrival and Departure), and a current Non-U.S. Passport. • One (1) document from the Registry’s list of Acceptable Identification Documents List proving date of birth. • One (1) document from the Registry’s list of Acceptable Identification Documents List proving your signature. Removing Melanie’s Activity Hold for Mass ID Cards Activity holds placed on a consumer’s record due to Melanie’s Law can be removed for the purpose of issuing a Massachusetts ID card.
