This seminar is designed for practitioners who want to go a step further. Attendees will examine the pathogenesis of pain, and how this translates into physical findings and patient complaints. Participants will gain in-depth knowledge about the pharmacology of analgesics and co-analgesics, as well as starting doses, titration strategies and monitoring strategies. Participants will practice putting this all together – starting with the patient complaint and physical findings, developing a hypothesis about the pathogenesis of the pain, selecting the most appropriate analgesic regimen based on initial presentation, monitoring patient response and adjusting the analgesic regimen. The afternoon will be devoted to advanced practice skills development in opioid dosing strategies. We will begin by discussing the rationale for opioid conversions, or rotation, and learn about a five step process, using an equianalgesic dosing table. Practitioners will learn how to initiate and titrate opioid regimens (up and down) in a wide range of patients including opioid naive patients (ranging from mild pain to acute onset severe pain), patients requiring rescue opioid therapy and patients with very high opioid requirements. Participants will learn about the intricacies of dosing fentanyl and methadone, patients receiving patient-controlled analgesia and special considerations for patients receiving neuraxial opioid therapy. This is guaranteed to be an exhausting, high-yield learning experience! Palliative care providers are frequently required to help patients and families reach difficult care decisions. In many cases, the beneficiaries of palliative and hospice care are unprepared for these decisions and must be educated and brought to a position of informed consent in a short amount of time. The journey to a safe and healthy decision may require care providers to facilitate patients and families through conflict within and between themselves and the unfamiliar healthcare system. This program will provide key foundational skills to equip palliative care practitioners in facilitating the difficult conversations with patients and families in these crucial moments.
