PC04 We Honor Veterans Train-the-Trainer Program: Improving Access to Quality Care for Veterans and their Families
One in four dying Americans is a veteran. Many of these men and women have unique needs in the last months of life that surface as they face their mortality and reflect on their lives, including their service to our country. Over the past twelve years the NHPCO and the Department of Veterans Affairs have worked at the national, state and community level to improve access to quality end-of-life care for veterans and their family members. At this conference, NHPCO will unveil a national campaign to increase access and improve quality care for the millions of veterans living with a serious, life-limiting illness or condition. We Honor Veterans, a project of NHPCO and the Department of Veterans Affairs, invites you to join us in our efforts to learn more about the unique needs of veterans and enhance the capacity of hospices to serve those who have served our country. By recognizing a veteran’s unique needs, we can become better prepared to accompany America’s veterans and their families on their final journey. Participants will leave the training with a completed, detailed action plan to improve care provided to veterans in their hospice and/or entire state. Training materials, policies and procedures, outreach resources, clinical assessment tools and much more will be provided.
NHPCO will award travel stipends to two people in every state to facilitate their participation in this program. The travel stipend includes airfare (up to $500) and room and tax for two nights in the Omni Hotel, CNN Center. Go to www.nhpco.org/veterans to download a stipend application before July 12, 2010. Stipends are only available to people who attend the entire Preconference Seminar. Failure to attend the entire seminar will result in an invoice to recoup the travel stipend award. OBJECTIVES:
C Identify the unique needs of veterans in the last years of life C Describe the multiple models hospices can utilize to serve veterans C Delineate challenges hospices may encounter in working with
VA facilities and outline potential solutions to each challenge
C Develop an action plan to engage hospices, hospice staff and
the community in activities to improve care of veterans

This is a complimentary event for all Main Conference attendees. evening Preconference event • sunday, september 12 – 5:00 pm - 6:00 pm

Hearing the Voices of “The Soon Departed”
So often the focus in hospice is (appropriately) on what we can do to help the patient and family. But in our enthusiasm to help them, what can be overlooked is what the hospice patient can do for those of us who are left behind. What can we learn from them? In-depth interviews with 104 hospice patients receiving care through Alive Hospice in Nashville, Tennessee, provided inspiring insight into what really matters in life when viewed through the lens of the “soon departed.” Dame Cicely Saunders, the founder of St. Christopher’s Hospice outside London, England, said “the community needs the dying to make it think of eternal issues and to make it listen. We listened and heard.
OBJECTIVES:
C Discuss the perspective of hospice patients as they near the
end of life
C Examine your own personal attitudes toward living and
dying so that you will be better able to assist patients as well as to learn from them
C Design a process for assuring that the broad voice of the
patient is both valued and heard
Dr. Bob Fisher has previously served at universities in the roles of professor, business school dean and vice president of academic affairs and currently serves as president of Belmont University in Nashville (www.belmont.edu). He is a Fulbright Scholar and serves in numerous volunteer roles in support of Nashville’s public schools and as past chair of the Greater Nashville Area Chamber of Commerce. Judy Fisher has filled the roles of mother, middle school science teacher and, currently, the campus-wide coordinator of interior construction and exterior landscaping/lighting at Belmont University. Judy conducted the large majority of the interviews for this project. She is active in community service where she serves on several non-profit boards including the American Lung Association, the Nashville Opera and, most notably, the board of Alive Hospice. The Fishers have spoken to more than 100 groups on this topic including the Duke University End of Life Institute, Vanderbilt University resident physicians, and provided the opening keynote presentation for NHPCO’s 2008 conference in Dallas.
