Richard Raskin MD, FACP is a graduate of the University of California, Berkeley and Northwestern University Medical School and is board certified in internal medicine and rheumatology. He has extensive health plan experience with Sierra Health Services and UnitedHealth Group and has been actively involved with hospice since the 1980s. He has served on the Board of Directors of NHPCO and has chaired the NHPCO Managed Care Advisory Council since 2003. Dr. Raskin is currently Vice President of Medical Affairs for Genesis Health Care in Andover, MA. In this role he has participated in the development of a comprehensive advance care planning and end-of-life program applicable to all patients and residents of GHC facilities.
Creating the Continuum: The Health Insurer’s Perspective

Much of the current healthcare payment system is provider and episodespecific, encouraging “silo-ed” rather than coordinated care. Traditional medical and hospice care are usually reimbursed through two separate payment methodologies, without clinical or financial overlap. As a result, there is very little data demonstrating the efficacy or cost-effectiveness of offering the hospice model, as currently defined, to people earlier in their disease progression. This panel will discuss ways that hospices and other providers can collaborate with health insurers to offer palliative care and care management services now and in the future. Results from Aetna’s pilot program on concurrent care and Harvard Pilgrim Health Care’s Oncology Care Management program will be shared as well as ideas on what health care reform and other changes will bring to palliative care providers.
Randall Krakauer MD graduated from Albany Medical College in 1972 and is Board Certified in Internal Medicine and Rheumatology. He received training in Internal Medicine at the University of Minnesota Hospitals and in Rheumatology at the National Institutes of Health and Massachusetts General Hospital/ Harvard Medical School and received an MBA from Rutgers. He is a fellow of the American College of Physicians and the American College of Rheumatology and Professor of Medicine at Seton Hall University Graduate School of Medicine. He is past chairman of the American College of Managed Care Medicine. Dr. Krakauer has more than 30 years of experience in medicine and medical management, and has held senior medical management positions in several major organizations. He is responsible for medical management planning and implementation nationally for Aetna Medicare members, including program development and administration.
Catherine McFadyen MS, RNC, ANP In the course of a diverse career, encompassing various aspects of the clinical and management sides of healthcare, Catherine McFadyen has proven to be a skilled and thorough-minded professional and expert in her field. Catherine has used her skills as a Registered Nurse Practitioner and Cardiac Critical Care Specialist to assist in the development of programs at several healthcare facilities throughout Massachusetts. Catherine is currently the Director of the Care and Disease Management Programs at Harvard Pilgrim Health Care, where she has assisted in the development of such programs as the Prepared for Care and Oncology programs. In addition to being published, Catherine has worn the cap of presenter, teacher and consultant utilizing her knowledge and passion for healthcare to educate others.
John Mach MD holds a bachelor’s degree from St. Thomas University and an MD from the University of Minnesota Medical School. He is board-certified in Internal Medicine and received a Certificate of Added Qualifications in Geriatric Medicine in 1988. John is currently the Chief Medical Officer of XLHealth Corporation, a leading participant in the Medicare Special Needs Plan (SNP) program. XLHealth coordinates individualized healthcare benefits and well-being services for Medicare beneficiaries who are chronically ill. Prior to joining XLHealth, Dr. Mach served as Chairman of Evercare, a division of UnitedHealth Group that specializes in coordinating care for people who are frail, disabled, chronically ill or at the end of life. Dr. Mach joined Evercare in 1996 as its Chief Medical Officer and proceeded to serve as Executive Vice President followed by six years as Evercare’s Chief Executive Officer. Concurrent to his position as CEO of Evercare, Dr. Mach also was the Chief Medical Officer of Ovations, the UnitedHealth Group segment serving Americans over 50.
