Before they get too far down this diversification path, attendees should carefully consider whether diversification is the optimal strategy, given their organization’s situation and market conditions. From a financial perspective, the rationale behind the expansion across the care continuum is to increase the long-term financial viability of the organization. The speakers will present a process to analyze diversification versus alternative investments of management time and resources to accomplish this same goal. This session will address how community-based palliative care services can respond to patient preferences for care at home and policy initiatives designed to avoid unnecessary re-hospitalizations. Case studies will briefly outline three service delivery models. A structured work plan will help participants frame next steps, including identification of mechanisms to facilitate smooth transitions between inpatient and community-based care. Specific strategies, tools and resources will be offered to help you engage staff to enhance your quality improvement efforts and ultimately exceed the expectations of your patients and families. 2F Implementation of a Hospice Respiratory Therapy Program that Improves Quality, Provides Continuity and Decreases Costs Mary Jo Quinn MS, ARNP, ACHPN, HPC Healthcare, Inc, Temple, FL 2C Serving Your Community by Building and Sustaining a Comprehensive Palliative Care Continuum Susan Bruno MSW, LCSW, Suncoast Institute, Clearwater, FL Stacy Orloff EdD, LCSW, Suncoast Hospice, Clearwater, FL Palliative care programs are gaining in popularity as healthcare systems recognize their integral role in developing a continuum of services ranging from chronic care to end-of-life care. Learn how one community-based hospice and palliative care provider developed a functioning continuum of palliative care programs based on age, location and patient/family need. A continuum of programs will be reviewed which represents the community needs from pre-birth through old age. Hospices routinely care for patients with end-stage cardiopulmonary diseases. Although more than half of hospice patients are reported to experience dyspnea at some point in their hospice care, data regarding the utilization and cost of respiratory therapists in hospice programs has not been reported. Advances in COPD care include more frequent use of bi-level positive airway pressure (BiPAP) to better treat dyspnea and fatigue, as well as an alternative to intubation when mechanical ventilation is needed. The latter has resulted in increased referrals of patients who are unable to be weaned from BiPAP. HPC Healthcare has established a successful hospice respiratory therapy program that has increased access to care and improved quality of life, while decreasing costs.
