EDUCATION HIGHLIGHTS
PACE Site Tours
Optional Site Visits of PACE Programs Conference attendees have the unique opportunity to visit one or two of the 72 PACE (Program of All-inclusive Care for the Elderly) providers in the nation. Tours are on August 4, 2010 from 10:00 am-1:00 pm (depending on traffic). Cost is $40 per person and includes bus transportation, a tour of the facilities, information about the PACE model and ample time for questions and answers. Please note: The maximum number of attendees touring either site is 20 people. If NHPCO receives more than 20 registrations for the site visits, a waiting list will be established and the tour may be split between the two sites. Registrants will be notified and asked to select their preferred site if that occurs. NHPCO reserves the right to cancel this event if the minimum number of registrants is not reached. Attendees will be notified two weeks prior to the event if cancellation is necessary. See page 25 to register.
Upham’s Elder Service Plan: Program of All-inclusive Care for the Elderly (PACE) One of six in Massachusetts implementing the PACE model of care, Upham’s Elder Service Plan (UESP) serves central Boston and is sponsored by Upham’s Corner Health Center, a community health center in the Dorchester neighborhood of Boston. The mission of UESP is to provide comprehensive and continuous health and social services that will support the independence and nurture the spirit of disabled elders; in cooperation with community and family caregivers, UESP provides interdisciplinary care with compassion, respect for dignity and sensitivity to cultural differences. UESP provides end-of-life care through its own teams, whether that care is delivered in a participant’s home or at one of its network nursing homes. The PACE Center is the hub of the PACE service delivery model, integrating a social day program with a fullservice multidisciplinary clinic. UESP opened its first PACE Center in 1996 in the Savin Hill area of Dorchester and its second PACE Center in 2008 in Dudley Square, the heart of the Roxbury neighborhood of Boston. UESP centers, staff and participants reflect the diversity of Boston’s communities. At our PACE Centers, you may encounter four different languages, hear varied music and observe participants (and staff) dancing to rhythms from around the world. Often, the PACE Center itself is the most valuable intervention for the health, spirit and well-being of our participants. www.uphamselderserviceplan.org
East Boston Elder Service Plan Like hospice, PACE is capitated to provide an array of services. For PACE, those services include all Medicare A, B, and D services, all covered service under Medicaid and other services as may be approved by the interdisciplinary team. The East Boston Elder Service Plan (EB ESP), along with more than 70 other PACE programs nationally, shares the mission of UESP. All PACE programs are built on an interdisciplinary approach to care that is best observed in our PACE Day Health Centers but is provided in whatever setting best suits our participants’ needs — home, hospital, nursing home or PACE Center. The East Boston Elder Service Plan (EB ESP) has been in operation for 20 years. One of the first PACE replication sites, the East Boston Elder Service Plan serves a tightknit, multi-ethnic community located “across the harbor” from downtown Boston. EB ESP began its operations as a partnership with its local community development corporation, with a goal of providing housing and round-the-clock access to care, along with the more traditional PACE Day Health Center Services. Its first PACE Day Health Center — a full ambulatory clinic combined with rehabilitation gym, social work and nutritionist offices and a social center — was co-located with subsidized elderly housing, a model that has since expanded to three additional housing sites.
