NHPCO 2010-12 Strategic Plan A C C E S S Strategic Pillar—Access Goal: Ensure all people are offered end-of-life care as part of a seamless continuum of care; actively work to reduce disparities in the provision of end-of-life care. Objectives 1. Facilitate the creation of a consensus agenda to expand patient/family options for chronic/palliative/end-of-life care 2. Create NHPCO’s “Advancing End-of-life Care Access Awards” program, which identifies rewards and promotes programs that have measurably increased timely access to hospice Key Initiatives a. Convene key stakeholders to develop a consensus agenda on the future of end-of-life care b. Promote NHPCO’s consensus agenda on the future of end-of-life care a. Solicit underwriting b. Package and promote best practices Current Activities to Support This Strategic Area • Disease-specific resource development and partnerships (e.g., chronic kidney disease was the focus in 2009) • Population-specific outreach guides • Provider-specific partnerships (finalizing Adult Day Service) • Language-specific consumer resources • ChiPPS – Standards, Facts and Figures, online educational curriculum and compendium • Special projects for veterans, including a focus on rural and homeless veterans, and equipping hospices to provide services to veterans • Development of resource materials for end-of-life care in correctional settings
