effusions (all received mepolizumab). One patient's disease
status could not be determined because of a lack of follow-up
data, and 1 patient underwent a tympanoplasty 9 months after
starting mepolizumab.
Conclusions: In conclusion, EOM is a rare disease that otolaryngologists should include in their differential diagnosis, especially in refractory cases. Anti-IL-5 agents show efficacy in
treating EOM, and prospective multicenter clinical trials are
needed to further characterize the effect of anti-IL-5 therapies.

Evaluation of Eustachian Tube
Dysfunction and Treatment Outcome by
Chinese Version of ETDQ-7 and PHI-10

Objectives: Obstructive Eustachian tube dysfunction
(OETD) and patulous Eustachian tube dysfunction (PETD)
are 2 distinct forms of Eustachian tube dysfunction (ETD).
Currently, there is no "gold standard" to diagnose the disease.
The 7-Item Eustachian Tube Dysfunction Questionnaire
(ETDQ-7) is the only subjective tool for the assessment of
aural symptoms in ETD patients, and the Patulous Eustachian
Tube Handicap Inventory-10 (PHI-10) is suitable for evaluating the severity of life handicap in PETD patients. Until now,
the efficacy of the PHI-10 for OETD has not been assessed by
any research.
Methods: We prospectively enrolled 57 patients diagnosed
with OETD and 51 healthy individuals in this study. After
2-staged translation of the ETDQ-7 and PHI-10 to the Chinese
version, 2 scales were completed by patients in OETD and
control groups. Cronbach a coefficient was used to evaluate
the internal consistency and receiver-operating characteristic
(ROC) curve calculated the discriminant validity.
Results: The Cronbach a coefficients for ETDQ-7 and PHI10 were .859 and .966, respectively. The test-retest reliability
coefficient for the total scale was determined as 0.925 (ETDQ7) and 0.982 (PHI-10). The area under the curve in ROC analysis was 0.977 for ETDQ-7 and 0.996 for PHI-10, which
confirmed their excellent discriminant validity toward the
healthy control group. Three OETD patients received balloon
dilation of the Eustachian tube successfully, and the scores of
both ETDQ-7 and PHI-10 were decreased after the treatment.
Conclusions: According to the results, both ETDQ-7 and
PHI-10 showed excellent sensitivity and specificity for diagnosis and screening of OETD. For OETD, we recommend
application of both scales in evaluation of the disease severity
and treatment outcome.
Functional Symptoms in Otology: A
Retrospective Review of 300 Consecutive
Adult Patients Referred to Otology Clinic

Objectives: Functional otologic symptoms include tinnitus,
imbalance, otalgia, and aural fullness in the absence of active

ear disease. They occur because of complex changes in neurocircuitry involving limbic and sensory systems. The burden of
functional disorders on the otology clinic is yet to be quantified. The aim of this study is to identify the proportion of new
adult referrals that present to an otology clinic with functional
disorders and propose management pathways.
Methods: The clinic notes of 300 consecutive adult patients
(January 1, 2019 to August 21, 2019) were retrospectively
reviewed. All were new referrals seen by 1 otology consultant.
Data were collected on patient demographics, symptoms,
medical and psychiatric history, life stressors, examination,
audiology results, scan results, diagnosis, and treatment.
Results: A total of 35% (106/300) of patients were diagnosed with a functional disorder, with no findings on examination or investigation. Twenty-three percent of patients had
primary tinnitus, 9% functional otalgia, 8% aural fullness,
5% functional imbalance, and 3% auditory processing disorder; 25% of patients with functional symptoms described
associated life stressors. We propose a management pathway
including techniques to address causation (tinnitus retraining
therapy, relaxation, mindfulness, and cognitive-behavioral
therapy referral).
Conclusions: One in 3 referrals to otology are due to functional symptoms. Misinformation from internet sources causing anxiety may be driving a rise in new referrals. Understanding
complex neurocircuitry and the influence of the limbic system
on sensory processing is essential in managing these patients.
Clinicians should be wary of fitting functional symptoms into
a physical diagnosis, as this can exacerbate the problem and
lead to unnecessary treatment.
Genetic Study in Idiopathic Sensorineural
Hearing Loss in Adults

Objectives: Diagnosis of genetic hearing loss is challenging,
given its extreme genetic and phenotypic heterogeneity, particularly when diagnosed in adulthood. Therefore, late-onset
genetic sensorineural hearing loss (SNHL) may be underestimated. To date, most cases of adult SNHL are classified as
idiopathic, underestimating an underlying genetic pathogenic
variant. The aim of this study is to evaluate the utility of a
next-generation sequencing (NGS) gene panel in the etiological diagnoses of adult onset SNHL.
Methods: Adults with idiopathic SNHL were recruited from
the otolaryngology department at Marques de Valdecilla
University Hospital (Spain). Familial hearing loss and medical history were collected. Pure tone audiometry was performed to determine the severity and pattern of hearing loss.
An NGS gene panel (OTOgenics), composed of 199 genes
related to SNHL, was performed in all of them.
Results: A total of 55 individuals were included in the study,
39 (71%) women and 16 men (29%), with an average age at
diagnosis of 41 years. All had bilateral and symmetric
