Conclusions: This study reveals that smoking significantly
increases major and minor complication rates after microvascular reconstruction surgery performed by otolaryngologists.
Future studies are required to determine the impact of smoking cessation on mitigation of these observed adverse events.
Emergency Cricothyrotomy Simulation:
Using a Porcine Model for Resident
Education

Objectives: To (1) provide otolaryngology residents with
practice in obtaining a rapid surgical airway in a porcine
model; and (2) determine if prior resident experience with performing tracheostomy increases speed of performing successful cricothyrotomy.
Methods: Adult porcine larynges were obtained in accordance with university policies. All of the 13 participating residents were provided with an unloaded scalpel and a soft-tissue
instrument tray. Each was timed from the start of scalpel loading through completion of cricothyrotomy. Attendings confirmed successful completion. The total number of prior
tracheostomies per resident was compared against their speed
of successful completion.
Results: A total of 13 otolaryngology residents participated
(postgraduate year 1-5). The time for successful cricothyrotomy ranged from 57.38 seconds to 4 minutes 21 seconds. All
residents were successful. The total number of prior tracheostomies ranged from 1 to 82. Time to perform successful porcine cricothyrotomy did not directly correlate with prior
number of tracheostomies. In general, however, more experienced residents performed more rapid porcine cricothyrotomies. Of note, 1 of the participating residents successfully
performed a cricothyrotomy in the emergency department the
night after completion of the course.
Conclusions: Speed of successful porcine cricothyrotomy
did not directly correlate with prior experience, but more rapid
cricothyrotomies were generally performed by more experienced residents. Practice of cricothyrotomy in a nonemergency setting is a beneficial teaching tool for this life-saving
technique frequently performed by otolaryngology residents.
An Exploration of Otolaryngology in the
Reddit Community

Objectives: Patients increasingly turn to online resources for
medical information. Reddit, a social media network of online
communities, is the sixth most visited website in the United
States. Despite its popularity, few studies investigate the use of
Reddit as a health care resource. We aimed to characterize topics and intentions of otolaryngology-related posts on Reddit.
Methods: We compiled a list of 44 terms representing key
conditions and procedures included in the American Academy
of Otolaryngology-Head and Neck Surgery Clinical Indicators

and Clinical Practice Guidelines. We searched each term on
Reddit to identify "subreddits" (forums dedicated to specific
topics) pertinent to otolaryngology. Subreddits that were not
active in the past year and those with fewer than 50 posts were
excluded. Themes were identified from the top 50 posts from
each subreddit as sorted by popularity.
Results: Of 27 otolaryngology-related subreddits identified,
23 met the criteria for analysis. The 3 most popular forums
were Ear Rumblers Assemble, Hypothyroidism, and Reddit's
Tinnitus Community. Of 1150 posts in all subreddits, 67%
sought health advice, 20% shared users' personal experiences,
4% linked to external sites, and 2% pertained to medical education or research. An additional 7% of posts were not related
to otolaryngology. Those seeking health advice asked about
symptoms and diagnosis (55%), treatment (33%), and perioperative concerns (12%).
Conclusions: Focused analysis of otolaryngology-related
posts on Reddit may contribute to identifying and addressing
the informational needs of patients seeking care for common
otolaryngic conditions. Understanding social media content
may also facilitate clinicians connecting patients with otolaryngologic disease-specific support forums.
Finding a Market Alternative to Zantac:
Comparison of Adverse Events of 4 H2
Blockers Using the FDA Adverse Event
Reporting System

Objectives: The H2 blocker Zantac and other ranitidine products
were voluntarily recalled by the Food and Drug Administration
(FDA) because of the presence of N-nitrosodimethylamine. Using
the FDA Adverse Event Reporting System (FAERS), a database
that compiles adverse events attributed to approved drugs, to look
at side effect profiles of other popular H2 blockers used for heartburn and gastroesophageal reflux disease, we can help determine
a safe market alternative to ranitidine with a better understanding
of the associated risks.
Methods: Using FAERS data collected between 1977 and
2019, we studied 38,609 adverse events attributed to Zantac,
Axid (nizatidine), Tagamet (cimetidine), and Pepcid (famotidine). Three categories of reactions were noted to be both common and unexpected in each drug by 2 reviewers: those involving
the central nervous system (CNS), cardiovascular system, or
allergic hypersensitivity. Relative frequencies of total reactions
per category were compared to determine any significant similarities or differences in the drugs' side effect profiles.
Results: CNS effects comprised the greatest proportion of
total complaints of the categories evaluated. CNS complaints
were 14.48% of reactions to Zantac, 14.40% to Pepcid,
14.19% to Axid, and 9.99% to Tagamet. Axid had the highest
proportion of allergic hypersensitivity reactions at 18.43%.
Tagamet had the lowest proportion of all reactions, with CNS
reactions as stated previously, 2.59% cardiac, and 3.80% allergic hypersensitivity.
