parotid malignancy, (4) management of the facial nerve in the
setting of malignancy, and (5) complication rates and management. This session will allow for audience interaction and
appeal to general otolaryngologists, academicians, and trainees, in addition to the head and neck oncological surgeon. This
2-hour panel presentation will cover the topics above using a
combination of didactics, audience-response queries, and
case-based examples.
Outcome Objectives: (1) Use an evidence-based framework
to determine appropriate use of capsular dissection in benign
parotid disease. (2) Review the indications for superficial vs
total parotidectomy as well as the use of neck dissection in
parotid malignancy. (3) Evaluate a data-based approach for
facial nerve management in malignancy, and the prevention
and management of complications.
Debating Fiscal Controversies in Head and
Neck Reconstruction

Session Description: There is growing concern regarding the
fiscal sustainability of health care systems, with an increasing
need to better understand health care economics. Efforts to
limit health care spending and provide efficient use of health
care dollars have become paramount. This panel is designed to
allow experts in the field to debate controversial fiscal topics
in head and neck reconstruction. These include the use of
3-dimensional modeling for reconstruction, free flap monitoring methods, and free flap reconstruction following palliative
surgical ablation. Are these practice patterns cost-effective and
justifiable or are they going to be increasingly difficult to
defend? This panel will provide the most up-to-date data and a
review of the current literature on these topics, providing participants with the resources necessary to determine for themselves how they would answer these challenging questions.
Outcome Objectives: (1) Review the controversial topics in
head and neck reconstruction. (2) Analyze the data in support
of certain practice patterns. (3) Identify which practice patterns are not cost-effective.
Enhanced Recovery After Surgery:
Optimizing Perioperative Patient Care

Session Description: In recent years there has been increased
scrutiny of specific postoperative metrics by hospital administrators and payers. Surgeons have been asked to minimize
hospital length of stay, readmission rates, and postoperative
complications for their patients both for quality and financial
reasons. Enhanced Recovery After Surgery (ERAS) programs,
whose ultimate goal is expedited recovery of patients, has
implications for these issues. ERAS relies on interdisciplinary
coordination that optimizes patient nutrition, antibiotic

prophylaxis, thromboprophylaxis, and pain control, among
other factors, through standardized and evidence-based care
pathways to achieve early mobilization and faster recovery.
Importantly, these protocols use multimodal analgesia strategies to minimize opioid use. These strategies have obvious
implications for patient safety and quality improvement as
well as for addressing our national opioid crisis. This panel
discussion will focus on informing otolaryngologists about
the emerging literature on ERAS in head and neck surgery and
opioid-sparing multimodal analgesia as an essential component. Speakers will describe the benefits of ERAS and strategies for the enactment into practice for patients undergoing
head and neck surgery. Audience members will have the
opportunity to choose specific ERAS interventions for further
discussion of practical recommendations for adoption.
Outcome Objectives: (1) Describe the rationale and role of
enhanced recovery pathways in head and neck surgery. (2)
Identify multimodal analgesia as an alternative to opioid analgesia in head and neck surgery. (3) Understand strategies for
introducing enhanced recovery and multimodal analgesia into
practice.
HPV Status: Updates in Head and Neck
Cancer

Session Description: This is an interactive panel presenting
the most up-to-date evidence of the role of human papillomavirus (HPV) in head and neck cancer incidence, treatment, and
prognosis with a focus on how this affects the practice of the
general otolaryngologist. The latest evidence and guidelines
for testing of HPV status in suspected malignancies of the
upper aerodigestive tract will be reviewed. Updated evidence
on the utility of HPV in the workup and management of the
unknown primary neck mass will be emphasized. The impact
on prognosis (and staging) will be discussed with a focus on
recent controversy surrounding HPV status on prognosis for
nonoropharyngeal subsites. Recent de-escalation trials in the
management of HPV-associated unknown primary and oropharyngeal subsites will be discussed to provide a glimpse
into the future management of these patients. Recommendations
on how the clinician should converse with patients (and families) regarding the diagnosis will be highlighted while re--
viewing evidence on how this affects patient/partner sexual
behavior. Vaccination recommendations will be bolstered
with evidence from recent studies demonstrating no impact on
sexual behavior of teens receiving the vaccine.
Outcome Objectives: (1) Interpret the evidence on HPV testing for neck masses, unknown primary cancers, and nonoropharyngeal head and neck cancers. (2) Demonstrate clinician communication with patients (and families) regarding the
diagnosis of an HPV-associated malignancy. (3) Recognize
the effectiveness of HPV vaccination and review the impact of
vaccination on sexual behavior in teens.
