improved with conservative management. Per our literature
review, this is the first example of pediatric SNHL caused by
lightning injury and assessed with audiometry.
Utility of Temporal Bone Computed
Tomography in Pediatric Patients

Objectives: The clinical utility of temporal bone computed
tomography (CT) imaging in infectious workup of pediatric
patients remains unclear. Our objective was to examine the
indications, findings, and clinical outcomes associated with
pediatric CT temporal bone scanning, with the overall goal of
developing an algorithm to reduce the number of clinically
uninformative CT temporal bone scans.
Methods: We performed a retrospective review of all CT
temporal bone studies performed on patients younger than
18 years in the emergency department or inpatient settings from
2012 to 2016. Data were collected regarding patient demographics, the indication for imaging, radiographic findings, and
clinical outcomes. Fisher exact test or Student t test was used to
evaluate for significant differences between groups.
Results: We identified 99 CT temporal bone studies that
were performed in 91 unique patients. The mean age of the
population was 8.6 years, and 59% of the patients were male.
Most studies (68%) were performed in association with an
inpatient admission. In this patient population, CT temporal
bone studies were most commonly obtained to evaluate for
complications of acute otitis media (59.6%) or as part of a
trauma evaluation (37.4%). Most studies (80%) in this population did not lead to surgical intervention or change in clinical
management.
Conclusions: Overall, the present data indicate that CT temporal bone studies are performed relatively infrequently in
pediatric patients at our institution. Further analysis will identify clinical findings associated with CT temporal bone studies
in this population, which may identify opportunities to avoid
unnecessary studies in low-risk patients.
Very Early Enzyme Replacement Therapy
for Infantile-Onset Pompe Disease
Contributes to Better Hearing Outcomes

Objectives: Deficiency of the lysosomal enzyme acid alphaglucosidase (GAA) may cause infantile-onset Pompe disease
(IOPD), which is a rare but fatal disorder without treatment.
Improvement of survival time and neurologic deficits was
already known by application of enzyme replacement therapy
(ERT) with recombinant human GAA. However, with regard
to hearing outcome, how early the timing for application of
the ERT is still unknown.
Methods: In Taiwan, Pompe disease screening has been
included in the nationwide newborn screening system since

2008. This study included 20 children who were referred to
Taipei Veterans General Hospital and confirmed diagnosis as
IOPD between January 1, 2008, and January 31, 2014.
Results: The average days of initiation of ERT treatment
was 10 days after birth. In our study, 3 children had bilateral
mild to moderate hearing impairment (26.7-45 dB); 1 child
had single-ear mild hearing impairment (33 dB) according to
the American Speech-Language-Hearing Association hearing
classification systems. Our study group had better hearing
outcomes and early timing of ERT (25% hearing impairment;
average time of initiating ERT, 10 days) compared with studies published in 2010 (90.9%; 2.92 months), 2012 (90%; 3.06
months), and 2015 (60%; 16 days).
Conclusions: Early diagnosis and then early initiation of
ERT may contribute to better hearing outcomes. Very early
ERT possibly has better hearing result than early ERT.

Assessing the Efficacy and Safety of Balloon
Sinuplasty in Elderly Chronic Rhinosinusitis

Objectives: The efficacy and safety of balloon sinuplasty
have been increasingly studied among adult and pediatric
populations but have very seldom been investigated in geriatric populations. Geriatric populations present unique considerations and potential complications during surgery. Balloon
sinuplasty offers both minimal invasiveness and shorter procedure time compared with traditional surgical interventions
and can serve as a safer option for surgical management in
elderly chronic rhinosinusitis patients.
Methods: A retrospective chart analysis was conducted on
patients (n = 62) aged 75 and older who elected to have balloon sinuplasty for treatment of chronic rhinosinusitis resistant to medical therapy from 2016 to 2019 in a clinical setting.
A Sino-Nasal Outcome Test (SNOT-22) questionnaire was
answered before and after the procedure to assess the alleviation of symptoms. In addition, intraoperative and postoperative complications were recorded.
Results: The average preoperative SNOT score was 47.2,
and the average postoperative score was 19.7. The percentage
difference between the 2 scores was 82.2%, with a 58.3%
reduction in symptoms (P < .05). Only 3 patients (4.8%) had
postoperative complications, all of which were epistaxis
related. Two of these episodes required packing, and the other
was resolved with simple pressure placement.
Conclusions: Balloon sinuplasty proved to be not only effective but also a relatively safe procedure for the alleviation of
chronic rhinosinusitis symptoms in the elderly. By reducing
the invasiveness of traditional sinus surgery through balloon
dilation technology, surgical complications can be mitigated
while still providing significant relief of symptoms and
improved quality of life.
