Methods: A retrospective review of SNGPCs at a single
institution was conducted. Patient data, operative data, and
postoperative information were collected. These were compared with pooled data from published reports retrieved using
5 literature databases.
Results: Seven SNGPC patients were identified and compared with 142 literature patients. The most common presenting symptoms in both series were nasal obstruction, nasal
discharge, and epistaxis. In our retrospective series, all
7 patients underwent complete endoscopic resection. Two
underwent radiation treatment. No recurrences occurred over
a mean follow-up period of 40.8 months. In the literature
series, 70.4% of patients underwent endoscopic resection.
Neoadjuvant (7.7%) and adjuvant (6.3%) treatment were
infrequently reported. Eleven recurrences occurred at a mean
time of 48.3 months following surgery. α-SMA, CD34, and
vimentin were the most frequently reported immunohistochemical markers used for diagnosis.
Conclusions: Complete endoscopic resection with negative
margins is an effective treatment for SNGPC. Markers including α-SMA, CD34, vimentin, desmin, muscle-specific actin,
factor XIIIa, cytokeratin, and S100 help differentiate SNGPC
from other vascular sinonasal tumors.

Evaluation of Outcomes Following
Endoscopic Pituitary Surgery Based on
Physician Specialty

Objectives: Otolaryngologists (ENTs) and neurosurgeons currently perform most endoscopic pituitary surgery cases. This
study aims to ascertain the differences in outcomes between
ENTs and neurosurgeons performing pituitary surgery.
Methods: A statewide database was queried for patients
undergoing pituitary surgery in New York State from 2000 to
2014. Patients were stratified into neurosurgery or ENT based
on the operating surgeon's training specialty. Hospital and
patient demographic information were assessed using chisquare analysis. Outcomes of interest were 30-day mortality,
30-day readmission, prolonged length of stay (PLOS), and
postoperative complications. Multivariable logistic regression
was used to compare differences between the 2 specialties.
Results: ENTs performed 430 cases, and neurosurgeons performed 6821 cases of pituitary surgery. Patients operated on
by a neurosurgeon were more likely to be white, privately
insured, and in the highest income quartile (P < .05).
Neurosurgeons were more likely to operate in large hospitals
and teaching facilities as compared with ENTs (P < .05).
Surgeon and hospital volume were significantly higher for
neurosurgery than for ENT cases (P < .0001). After adjusting
for patient and provider characteristics, only cerebral vasospasm rates were significantly higher in ENT patients (odds
ratio 6.24 [1.43-27.19]). Thirty-day mortality, 30-day readmission, PLOS, and complications including cerebrospinal

fluid leak, intracerebral bleeding, hydrocephalus, sinusitis,
diabetes insipidus, and other endocrine derangements did not
significantly differ between specialties.
Conclusions: Outcomes of pituitary surgery performed by
ENTs and neurosurgeons were equivalent with the exception
of higher rates of postoperative cerebral vasospasm following
ENT operations.
Fipronil Upregulates Inflammatory Cytokine
and MUC5AC Expression in Human Nasal
Epithelial Cells

Objectives: Airway inflammation and excessive mucin production are pathophysiological characteristics of airway diseases. Fipronil, a pesticide, is extensively used in agriculture
and veterinary medicine worldwide. However, this compound
impairs immune function in nontarget organisms. The present
study aimed to evaluate the effect of fipronil on proinflammatory cytokine and mucus production and signaling pathways
in human primary nasal epithelial cells.
Methods: The effect of fipronil on proinflammatory cytokine and MUC5AC expression and the signaling pathway of
fipronil were investigated using real-time polymerase chain
reaction, enzyme immunoassays, immunofluorescence, and
immunoblot analysis with specific inhibitors and small interfering RNA.
Results: Fipronil treatment increased proinflammatory
cytokine interleukin (IL)-1β, IL-6, IL-8, and MUC5AC
expression in human primary nasal epithelial cells. It also
induced phosphorylation of extracellular signal-regulated
kinase 1/2 (ERK1/2) mitogen-activated protein kinase
(MAPK), p38 MAPK, and nuclear factor kappa-light-chainenhancer of activated B cells (NF-κB). MAPK and NF-κB
inhibitor treatment significantly inhibited increases in IL-1β,
IL-6, IL-8, and MUC5AC expression. Ex vivo data confirmed
that fipronil-induced MUC5AC expression occurs through
ERK1/2, p38, and NF-κB signaling pathways in nasal inferior
turbinate tissue.
Conclusions: Fipronil induced proinflammatory cytokine
IL-1β, IL-6, IL-8, and MUC5AC expression via ERK1/2
MAPK, p38 MAPK, and NF-κB in human primary nasal epithelial cells.
