left nasal cavity, pushing the cartilaginous part of the nasal septum to the right and left inferior turbinate aside. Surgical intervention by removal of the osteoma in parts, with drilling of the
left nasal floor and septoplasty, was performed under general
endotracheal anesthesia. A 6-month follow-up showed no signs
of recurrence and normal healing of the nasal floor mucosa. The
histopathology results were compact bone osteoid osteoma.

Nasal Leishmaniasis: An Underestimated
Presentation of Mucocutaneous
Leishmaniasis

Objectives: Upper respiratory tract infections (URTI) are
one of the most frequent causes of the visits in the emergency
care. Most of them are viral and self-limited, but many patients
still seek medical attention for symptomatic relief and better
treatment of this condition. This study aimed to perform a retrospective analysis of the treatment of upper airway infections
in a hospital specializing in otorhinolaryngology and to evaluate the effectiveness of treatment, as well as to demonstrate
that most of these infections have a favorable evolution despite
the use of antibiotics.
Methods: Retrospective chart review of patients who
attended to emergency care unit presenting diagnosis of URTI
from January to December of 2018. We analyzed the treatment and consider it effective if patients have not returned in
the following 15 days. Treatment failure was defined as a second visit up to 15 days after the first with a diagnosis associated with the primary infection.
Results: We analyzed 22,480 charts of patients. Of these,
approximately 50% received oral corticosteroids, 20% nasal
corticosteroids, 9% oral corticosteroids and nasal corticosteroids, and 28% received antibiotics. A total of 2320 (10.3%)
returned within 15 days of the first visit. When comparing the
use of each medication to the reconsult, the only drug that
showed an association with a lower rate of return was the antibiotic; however, the difference between the percentage of
return in the antibiotic and nonantibiotic group was only 1.6%.
Conclusions: Most URTIs have a favorable outcome, and
although the study found an association between antibiotic
use and the lower rate of return, this difference was almost
insignificant; therefore, there is no reason for prescribing antibiotics indiscriminately for this condition.

Objectives: Leishmaniasis is caused by the protozoan
Leishmania parasites transmitted through the bites of infected
female phlebotomine sandflies, which feed on blood to produce eggs. There are 3 main forms of leishmaniasis-visceral,
cutaneous, and mucocutaneous-and only a small fraction of
those infected by leishmania parasites will eventually develop
the disease. A case of nasal manifestation of a mucocutaneous
leishmaniasis is presented.
Methods: A 46-year-old female patient presented with
irregular scaly erythematous plaques all over the body, with
a massive plaque involving both nasal cavities, sparing most
of the nasal mucosa. The treatment, evolution, and prognosis are discussed, and a brief review of the literature is
described.
Results: The patient was referred for an ear, nose, and throat
consultation for having a massive scaly plaque surrounding both
nasal cavities. This plaque caused a significant nasal obstruction
giving the patient poor respiratory quality. Other plaques were
present all over her body, and a skin biopsy was performed. The
result of skin biopsy revealed an infection by Leishmania parasites. The patient received 6-month treatment with intraconazole
200 mg per day and nasal hydration. The patient had a good
clinical course, with a decreased number and size of the plaques.
She improved her respiratory and life quality.
Conclusions: Classic visceral and cutaneous leishmaniasis
remains endemic in developing countries and is rarely
observed in developed countries. There is no prophylactic
medication, and it always requires treatment. Sores can result
in permanent scars and disfigurement. Treatment may reduce
their severity and is most effective when started before extensive damage occurs.

prescriptions for antibiotics (6.5 vs 6.0), decongestants (1.2 vs
0.8), and leukotriene inhibitors (1.4 vs 1.0).
Conclusions: Pediatric patients with CRS are more likely to
be diagnosed and managed by nonotolaryngologists. This is
associated with a lower incidence of surgery and increased
medication use. The reasons for this discrepancy need to be
explored. Increased involvement by an otolaryngologist may
help to optimize treatment for these patients.
