Avoiding Complications in Endoscopic Sinus
Surgery

Session Description: Endoscopic sinus surgery is performed
frequently by otolaryngologists with a high rate of success,
but complications can be devastating. This panel presentation
serves as a discussion of preoperative and intraoperative considerations and recommendations to minimize the risk of surgical complications. The format is a unique combination of
case-based presentations and discussions, in addition to a
review of the current evidence and knowledge base, equipping
the practitioner with several essential tools with which to stay
safe and avoid complications in endoscopic sinus surgery.
Our panel presentation includes a critical understanding of
preoperative computed tomography (CT) review, essential
intraoperative anatomic landmarks, image-guided navigation,
perioperative pain management, antibiotics, steroids, surgical
hemostasis, and the immediate management of surgical complications when they do occur.
Outcome Objectives: (1) Describe how to review a sinus
CT scan for high-risk anatomy and recognize critical intraoperative landmarks. (2) Implement intraoperative techniques to maximize hemostasis, surgical held visualization,
and perioperative medical management. (3) Develop familiarity with the immediate management of intraoperative
complications.
Big Data Research in Otolaryngology: Does
It Change Your Practice?

Session Description: We live in an era of an unprecedented
explosion in data. Over the past 70 years, the doubling time of
medical knowledge decreased from 50 years to just 73 days.
Use of big data is becoming increasingly prevalent in the otolaryngology literature. Between 2005 and 2016, there was a
10-fold increase in database publications, and only 18% made
clinical recommendations. Conclusions drawn based on analyses of big data sets are frequently difficult to interpret because
of unaccounted confounding factors, bias, and lack of actionable recommendations. With initiatives such as Reg-entSM, big
data will continue to play an important role in otolaryngology
research and quality improvement efforts. New technologies,
including artificial intelligence, will bring about new tools
to process and incorporate vast quantities of data into oto-
laryngology practice. This panel will review the major data
sets commonly used in otolaryngology research, including
the National Surgical Quality Improvement Program and
Medicare claims. Data collection methods, quality assurance

measures, and limitations of these data sets will be discussed.
Our panel will present common pitfalls of database research
from the perspective of a clinician reader to address the fundamental question: "Does this study change my practice?" The
panel will address the future of big data in otolaryngology,
including the use of natural language processing and machine
learning to enable otolaryngologists to care for their patients
using the best available evidence.
Outcome Objectives: (1) Develop a working familiarity with
commonly used large data sets, their limitations, and common
pitfalls in big data research. (2) Analyze and apply big data
studies to assess the impact of confounding factors and bias on
study conclusions. (3) Explain how artificial intelligence can
leverage data from large numbers of patients to support otolaryngology practice.
Can Simulation Be Used as a Tool to Assess
Competence of Aging Surgeons?

Session Description: There is ongoing controversy over how
to assess the performance of aging surgeons. There is a consensus that a mandatory retirement age (as is used in other
disciplines) would be inappropriate if applied to surgeons
since data show that most older surgeons remain competent
into their later years. Moreover, an artificial age limit would
unnecessarily reduce the surgical workforce. However, cognitive changes accompany aging in many individuals, including
surgeons, and data suggest that such changes are often subtle
and not recognized by the affected individual. A need exists
for some level of investigation to inform decision making.
Many authorities have suggested that formal testing be used to
assess the competence of older surgeons; however, the ideal
strategies to do so remain elusive. This panel discussion will
pose the question as to whether performance in standardized
simulation scenarios might provide a solution. Panelists will
include Brian Nussenbaum, director of the American Board of
Otolaryngology-Head and Neck Surgery; Ellen Deutsch, an
expert in simulation; Edward Maitz, PhD, an authority on the
use of neuro-psych testing to assess aging professionals; and
David Eibling, a "geriatric otolaryngologist." This panel will
address some of the challenges inherent in the assessment and
potential role of simulation. An emphasis will be placed on
discussion of what kinds of simulation might be useful, how to
develop and set standards, and what the implications for the
practitioner might be. The panel will briefly discuss so-called
high-stakes simulation testing and how the results of such testing could be used by certifying boards and credentialing
organizations.
Outcome Objectives: (1) Recognize the challenges organizations face when assessing performance of aging surgeons. (2)
List how testing using simulation might enhance the objective
measurement of cognitive functioning of aging surgeons. (3)
Describe the current status and possible future of so-called
high-stakes simulation in assessing surgeon performance.
