Coeliac disease
Burden estimates for coeliac disease are not separately specified within the GBD 2019
database, therefore we conducted a systematic search for population-based studies
reporting prevalence estimates, published since the year 2000, conducted in UEG member
countries. Detailed methods can be found in the Appendix.
We identified 44 studies that met our inclusion criteria. Of these, 6 identified cases via
medical records, and 38 identified cases via population-based screening. Most of the
screening studies provided prevalence estimates for biopsy-confirmed coeliac disease, only 6
provided estimates based on serology alone. Many screening studies excluded previously
diagnosed subjects to provide a prevalence estimate for unrecognised coeliac disease,
however some included previously diagnosed cases in their prevalence estimates. To
enhance comparability of the results, we extracted data to calculate unrecognised coeliac
disease prevalence if this information was available from the screening studies. Nonetheless,
methodological differences remained in terms of the serological tests used and the criteria
that were applied to confirm a diagnosis. The 44 included studies contributed 51 prevalence
estimates for children, adults or mixed age groups, across 18 countries.
Prevalence rates for biopsy-confirmed, unrecognised coeliac disease amongst the screening
studies that provided results for children ranged from <0.2% in the UK and Russia, to >2% in
Spain and Sweden, and for adults ranged from <0.2% in the UK, Germany and Greece, to >1%
in Finland and Sweden (Table 2; Table 3). Prevalence rates amongst studies that identified
cases via medical records tended to be lower, and the majority of the estimates were <0.2%
(Table 4). Coeliac disease is likely under-diagnosed due to mild and asymptomatic forms of
the disease.
A recently published systematic review found that within Europe and North America, higher
latitudes were significantly associated with higher coeliac disease seroprevalence - findings
potentially explained by a combination of genetic, environmental and social factors.17
Additionally, significantly higher prevalence rates of biopsy-confirmed coeliac disease
amongst children compared to adults have been reported by previous systematic reviews.18
Some of the studies we identified also reported increasing trends in prevalence rates over
time. For example, prevalence rates of coeliac disease identified via medical records were
found to have increased from 0.08% in 1996, to 0.18% in 2016 in a nation-wide study
conducted in Denmark, and increased from 0.33% in 2000, to 1.11% in 2015 amongst
adolescents in Israel. Recent systematic reviews of the global prevalence and incidence of
coeliac disease also report increasing trends over time for children and adults.18 19 20
