DISCUSSION
This report provides an international comparative analysis of the public health burden of
digestive diseases and cancers, and an assessment of the economic impact of digestive
diseases amongst UEG member countries. To explore the changing epidemiology of digestive
diseases and cancers, variations in the distribution of burden over time were examined by
digestive disorder, country, age and sex. Digestive disease specific measures of indirect and
inpatient health service delivery costs were estimated across Europe to highlight the
economic value of reducing the burden of digestive diseases. Furthermore, to provide
insights into evolving challenges and where the greatest health gains can be made, estimates
of the proportion of digestive disorder burden attributable to modifiable risk factors were
examined, as was the relationship between human development and the burden of digestive
disorders amongst European UEG member countries. Here we discuss some of the main
findings of the report.
Public health burden of digestive diseases and cancers
Estimates from GBD 20191 suggest that digestive diseases are common and contribute to
increasing levels of burden amongst UEG member countries, with the total number of
incident cases, deaths and prevalent cases for digestive diseases overall on the rise. The
number of prevalent cases has increased by over a fifth since the year 2000, and currently
over 300 million people suffer from digestive diseases across the member countries. When
accounting for population growth and ageing, for the member countries combined, agestandardised
incidence rates for digestive diseases overall have remained stable and agestandardised
mortality rates have decreased since the year 2000. Stable incidence and better
survival have thus resulted in an increase in age-standardised prevalence rates over time.
For the UEG member countries combined, estimated age-standardised incidence or
prevalence rates have increased since the year 2000 for several digestive diseases, including
chronic liver diseases, pancreatitis, gastroesophageal reflux disease, gastritis and duodenitis,
paralytic ileus and intestinal obstruction, appendicitis, and vascular intestinal disorders.
Health systems and wider society may need to be prepared to cope with increasing levels of
digestive disease burden, due to increasing prevalence of most conditions and increasing
incidence of some conditions.
Digestive cancers also exert a substantial burden across UEG member countries, and together
are responsible for over a third of the total number of cancer related deaths. Ageing
populations have contributed to rising numbers of incident cases and deaths, apparent for all
